is there a way to add a folder or a file in a .tbi image without re-imagint the whole system?

You can use TBIHd to mount it with write support, modify it, then do a changes only or new full backup. You can also create your own script that uses tbosdt to injects files you want modified after restoring.

Otherwise, no, the image can't be changed which is a requirement for many mission critical environments our software is used with.
