Tried PEN and CD in both Direct IO, and non Direct IO versions.

Only tried with secure boot Disabled, Still hung at the same place.

Progress is being made. Removed ssd/sata hard drive from system and installed in USB/sata adapter. (secure boot disabled) Bootit_UEFI started without problem. Installed on SSD. rebooted to ssd.
Blue Screen BOOTIT UEFI 1.00 (Screen header)
Files Utilities Help (apparent window header)

popup in middle of page with :
Startup (header)
Synchronization in Progress

Then system hangs.

Moved ssd from USB to SATA hard drive internal and rebooted with the same result as above.

It should be noted that the system always hangs, but sometimes before the Synchronization in Progress popup appears.
Additionally rebooting with the pen installation installed, results in the same hang as before Black screen with Starting...
Don't know if it was luck that it booted in the USB adapter, but thought it best to install rather than look at partitions as I did last night.

How did you disable Legacy? Did you disable CSM (Compatibility Support Module)?

In Asrock Bios, The CSM provides Legacy support. It is disabled inorder to run BIU Install in UEFI mode.
Secure boot mode authenticates some KEYS that Microsoft has created to indicate a validated operating system/application.

Maybe a silly idea but does BIU boot if there is no USB mouse plugged in?

Removed mouse. No change

Just for fun I tried the UEFI version of IFW. It works fine for a non-gui implementation.

What is the "UEFI version of IFW"?

Just as an academic exercise, I tried to convert my working EMBR HDD to GPT intending to then try BIU.

I haven't been able to find a way to convert it. I'm beginning to think I should consider that fortunate.

Tested BIU on the board ASUS P8Q77-M.
Everything works except for the mouse.
Although in Bootit BM mouse works...

+1. IFL (which is slightly faster than IFW), is a lot faster than IFD. If possible, I would also like to see IFL included with BIU instead of IFD.
